This article provides an overview of the QOF Vaccinations domain and the resources available to support vaccination delivery and contract monitoring. It will help you understand the QOF vaccination indicators, monitor progress throughout the year and identify the appropriate Ardens resources to support your practice.
Background
The NHS England QOF Vaccinations domain supports practices in improving vaccination coverage among eligible patient groups.
For 2026/27, the Vaccinations QOF domain focuses on identifying eligible patients, monitoring vaccination uptake, and ensuring vaccination activity is accurately recorded.
Vaccination Indicators | |
VI001 | Babies who have reached 8 months old in the preceding 12 months who have received at least 3 doses of diphtheria, tetanus, and pertussis. |
VI002 | Children who have reached 18 months old in the preceding 12 months, who have received at least 1 dose of MMR or MMRV between the ages of 12 and 18 months. |
VI003 | Children who have reached 5 years old in the preceding 12 months who have received a reinforcing dose of DTaP/IPV and at least 2 doses of MMR or MMRV between the ages of 1 and 5 years. |
VI004 | Adults who have reached 80 years old in the preceding 12 months, who received a shingles vaccine between the ages of 70 and 79 years. |

🔍 Monitor Achievement
The QOF Vaccination reports support contract monitoring by helping practices review progress against vaccination indicators and identify factors affecting achievement.
Outstanding
Identifies patients who have not yet met the relevant QOF indicator.
Use these reports to:
Review remaining QOF activity
Monitor progress towards achievement

Data Quality
Highlights issues that may affect QOF achievement, including:
Missing vaccination codes
Incorrect coding
Missing or incorrect dates
Reviewing these reports regularly helps ensure records accurately reflect completed activity and support correct QOF reporting.

📱 Contract Requirements
Childhood Vaccination Base Year Calculation
For the 2026/27 QOF Childhood Immunisations indicators (VI001, VI002 and VI003), practices must calculate Base Year Numerator and Base Year Denominator values using achievement data from the 2024/25 and 2025/26 QOF financial years.
Although CQRS will calculate and upload these values later in 2026, practices must initially calculate and enter the values before monthly QOF achievement reporting becomes available.
To obtain the required values:
Within CQRS, open the Reports screen
Select Achievement Summary
Select the required Financial Year from the drop-down list
Click Run Report
Expand Public Health Domain
Select Public Health Domain: Vaccination and Immunisation
Open Vaccination and Immunisation
The report displays the Numerator and Denominator values for each indicator
Repeat these steps for each year.
Once you have obtained the values for both financial years, calculate the Base Year Numerator and Base Year Denominator by taking the average of the two yearly values.
Average = (Sum of Yearly QOF VI Values) ÷ (Number of QOF Financial Years with Available Data)
The calculated values are then used when determining achievement for the 2026/27 QOF Vaccinations indicators.

Where should I go to manage vaccination activity?
The QOF Vaccinations resources are designed for contract monitoring. For identifying patients, managing recalls and recording vaccinations, use Childhood Immunisations and Adult Immunisations.
Which reports should I use to monitor QOF achievement?
Use the QOF Vaccination reports: Outstanding to monitor patients who have not yet met the indicators, Data Quality to identify coding issues affecting achievement, and Achieved to review completed indicators.
Why is a patient showing as Outstanding after receiving a vaccination?
This may be due to the vaccination activity not being recorded using the required coding or the record not yet reflecting the completed activity. Review the Data Quality reports for potential issues.
