This introductory webinar provides an overview of the Ardens LTC Recall System and how it supports GP practices to manage the full long-term condition (LTC) review pathway. The system enables practices to move beyond a simple invitation process, supporting a structured and proactive approach to planning, notifying, assessing, reviewing, and evaluating LTC care. Using a month-of-birth recall model, practices can coordinate reviews for patients with multiple conditions, reducing duplication and improving efficiency across the practice team. What the session covers

This session provides a high-level overview of each stage of the LTC pathway:

  • Plan: Using the Ardens reports to understand the upcoming LTC workload and plan reviews across the year.

  • Notify: Managing the invitation process using structured reports.

  • Assess: Preparing patients for review by collecting key observations, tests, or questionnaires in advance.

  • Review: Completing coordinated LTC reviews using the Ardens templates to address multiple conditions in a single appointment.

  • Evaluate: Monitoring progress, identifying outstanding reviews, and tracking outcomes using reporting tools.

Additional webinars will be delivered throughout the year. Practices will also have the opportunity to explore LTC updates during their annual Ardens training sessions.
