NCD & Continuity of Care
From April 2026, continuity of care is now a core PCN requirement.
Ardens Manager can be used to identify continuity of care need through multiple risk stratification tools (National Content subscribers only, see our support article). Once coded, Ardens alerts within EMIS Web and SystmOne flag the continuity of care need when opening the patient's record, helping staff take action during triage or consultation.
Ardens Manager also now includes a dashboard to monitor continuity of care delivery. This includes Continuity of Care Index (CoCI) and Usual Provider of Care (UPC) to track GP continuity and % of appointments where the patient was seen by usual or most frequent GP.
We are hosting two webinars on 'Implementing Continuity of Care using Ardens' to walkthrough these resources:
→ Register for Continuity of Care Webinar - SystmOne - Weds 6th May, 1-2pm
→ Register for Continuity of Care Webinar - EMIS Web - Thurs 7th May, 1-2pm
If you prefer self-guided learning, we also have a new Risk Stratification course on Ardens Academy.

2025/26 Financial Year Claims
The 2025/26 financial year has now ended, and year-end extraction processes are underway; QOF will start from today and NCD from the 5th April. Following extraction, practices and PCNs can review and query claims via CQRS. ICBs will process LCS claims separately, however, Ardens clinical system reports and LCS dashboards remain available to support this process.
Check Patient - Ardens Manager
The new Check Patient functionality on Ardens Manager can be used when reviewing end-of-year QOF, NCD, and LCS claims to:
Confirm patient eligibility within reports
Understand why patients have or have not been counted
Identify and resolve data quality issues
