This article explains how to use Ardens Manager to support population risk stratification using the QAdmissions® model. By the end of this article, you will be able to identify patients at increased risk of emergency hospital admission and use this information to support proactive care planning, prioritisation, and service delivery.
Background
QAdmissions® is a validated risk prediction tool designed to estimate a patient’s likelihood of emergency hospital admission over a defined period. The model uses a combination of demographic, clinical, prescribing, and lifestyle data to identify patients who may require additional support or proactive intervention.
Unlike population segmentation models such as Bridges to Health, QAdmissions® focuses specifically on predicting unplanned hospital utilisation risk.
It supports organisations to:
Identify patients at increased risk of emergency admission
Support anticipatory and preventative care planning
Prioritise proactive interventions and reviews
Inform multidisciplinary team (MDT) discussions and case management
Support resource and service planning
Risk scores are generated using coded clinical data and should be used to support prioritisation and service planning alongside clinical judgement.

Select Services from the left-hand panel
Select the Population Dashboard
Select the Risk Stratification tab
Locate QAdmissions® (Predicted)

Use within reports
QAdmissions® can also be applied within any report in Ardens Manager.
To access this:
Click View next to a report
Select the RISK tab

🏷️ Categories
Patients are grouped according to their predicted risk of emergency hospital admission.
Risk categories may include:
Low Risk
Moderate Risk
High Risk
Unknown Risk
These groupings help organisations identify patients who may benefit from proactive review, coordinated care, or preventative intervention.
🔎Use Segmentation
Target patients within reports
QAdmissions® can be applied to any existing cohort to support prioritisation.
To access this:
Click View next to a report
Select the RISK tab
Open the Filter panel
Open the RISK section
Select QAdmission - 1 year (Predicted)
Choose the required category or categories
Click APPLY
This updates the report view to show data within the selected segmentation group.

Use this to:
Focus on patients at highest admission risk within a defined cohort
Combine admission risk with clinical or operational reporting
Support MDT reviews and proactive interventions
Prioritise preventative care activity
Patient-level review
To view patient-level data:
Click View next to a report
Select the PATIENTS tab
Open Patient View for individual records

Within Patient View:
QAdmissions® risk information is displayed alongside other risk stratification tools
This allows comparison across multiple risk models
Supports proactive review and care coordination activity
